RAIB recently published its report of a class investigation into factors affecting safety-critical human performance in signalling operations on the UK national network. This investigation examined five categories of incident in which signaller decision-making is pivotal, and where the safety of the railway system is heavily dependent on those decisions. The investigation identified several common factors relating to workload, user centred design, competence management, experiential knowledge and organisational structure, and the report contains recommendations for Network Rail in all these areas.


Mark is an Inspector with the RAIB and has professional responsibility for human factors within the Branch. He led the class investigation and will describe some of the incidents that were considered, the analysis of causal factors that was undertaken, and the conclusions that were drawn.
